All California wholesale customers are required by law to have a valid Seller’s Permit and are required to fill out this Resale Certificate Form for our records in order to have Tax Exempt Status and conduct business with us.

I hereby certify that I hold a valid Seller’s Permit issued and pursuant to the Sales and Use Tax Law to engage in the business of selling tangible personal, property at FV Eyewear, Inc. which will be resold by me in the form of tangible personal property; that in the event of such property is used for any purpose other than retention, demonstration, or display while holding it for sale in the regular course of business, it is understood that I am required by the Sales and Use Tax Law to report and pay tax, measured by the purchase price of such property.

I have read all terms and conditions and certify that all information entered is true and FV Eyewear, Inc. reserves the right to deny the user the right to purchase products if the information entered is invalid or fraudulent.
